A couple of things:

If turned down for a raise, better question than "why?" is "what stood in the way?" Why often feels like a challenge.

Also if reason for turndown is performance, in addition to asking what can be done for next time, employee needs to request more frequent reviews. Performance issues should never be a surprise at an annual review.
