In your experience, how often does rescinding of offers occur and what method does a company employ to deliver the news (letter, email, call, combination)?

Another one is not letting people you plan to use as references know to expect a call. I didn't do this last June, the guy wouldn't return the potential hiring manager's calls, and the job went to someone else.

You forgot one! Lying about something can easily get your offer rescinded. I know someone who lied about his salary history--it was discovered during the background check.
