1.Interviewer is late and/or comes unprepared meaning has not read your credentials.

2.Reading interview questions from a template.

3.Projects and unprofessional demeanor.

4.Is continuously interrupted by phone calls or other matters.

here is just a thought...if you folks should run into this type of situation...do what the military calls a pre-emptive strike on these buffoons and tell them thanks but you are no longer interested in the position...why sit under the sword of damoclese waiting for the thread to snap?

I got an interview recently for a relatively "low-skills" job (since I haven't found a more technical job where I live) and I was actually given career advice by the HR person. In not so many words, the interviewer basically told me that it's not that they didn't want me, but that with my qualifications and experience I should just finish my degree ASAP and get a better job than the one I was applying for.

Unfortunately, my catch-22 is that I can't afford school right now, so I have to take whatever job(s) I can!

I would add that if you notice this happening in the interview, there are ways you can address concerns. In retrospect, the people who interviewed me for my current position exhibited several of these behaviors. If you listen attentively, and respectfully address their concerns, you can still have a shot at the job. It doesn't hurt to have a great "why I'm perfect for this" spiel that you can go into if at the end of the interview they remind you that they are looking at other candidates.
